Prescriber Tab

The Prescriber tab contains contact and Prescriber Notification form information about the primary prescriber associated to the patient. In Ontario, a secondary prescriber can also be added.

Select the applicable tab to learn about the Prescriber tab in your province.

Use the dropdowns below to learn about possible actions in the Prescriber tab.

Changes to the patient's primary prescriber does not write back to Propel Rx. If the primary prescriber needs to be changed in Propel Rx, do so in the Patient Folder Preferences tab.

To change the primary prescriber:

  1. Select the Change button. The Change Prescriber window opens.

  2. Search for and select the correct prescriber. Select the Select button.

The information that appears in the Prescriber Notification form can be selected in the Prescriber Communication section. This form must be sent to the patient's prescriber to indicate the medication review has been completed and inform them of the outcome of the review.

To complete the Prescriber Notification form:

  1. Select the reason for the communication.

  2. Enter any notes in the Additional Notes box.

To add a secondary prescriber:

  1. In the Secondary Prescriber(s) section, select Add. The Change Prescriber window opens.

  2. Search for and select the secondary prescriber.

  3. Repeat steps 1 - 2 until all secondary prescribers are added, if required.

To remove a secondary prescriber:

  1. In the Secondary Prescriber(s) section, select Remove for the prescriber that needs to be removed.

  2. Repeat step 1 until all required secondary prescribers are removed, if required.

The Healthcare Provider Notification form must be sent to the patient's prescriber to indicate the medication review has been completed and inform them of the outcome of the review.

If the Print option is selected, a copy of the Healthcare Provider Notification will print for each prescriber when the review is completed. One copy of the patient's Personal Medication Record will also print.

If completing a Diabetes Annual or Diabetes Follow-Up Medication Review, the Diabetes Education Take-Home Summary form will also automatically be printed. As required by the ministry, this form must also be sent to the patient's prescriber. To print the Diabetes Education Checklist, select the Include Diabetes Education Checklist checkbox. The checklist will automatically be populated with the information entered in the Diabetes Education tab of the medication review.

When cloud fax is used and the prescriber has a fax number, the Fax option will automatically be selected. Forms should only be faxed if an electronic signature is captured, otherwise please print and sign the forms and then fax them manually.

The fax transaction is tracked and if a successful status is received from the cloud, a row will be added to the Patient Folder History tab. You can print the Care History list as evidence of the fax confirmation for auditing purposes. For more information, see Fax History.

If cloud fax is not used or the prescriber does not have a fax number, the Print option will automatically be selected. When the Print option is selected, a copy of the Healthcare Provider Notification form will print for each prescriber when the review is completed. One copy of the patient's Personal Medication Record will also print.

If completing a Diabetes Annual or Diabetes Follow-Up Medication Review, the Diabetes Education Take-Home Summary form will also automatically be faxed or printed. As required by the ministry, this form must also be sent to the patient's prescriber. To fax or print the Diabetes Education Checklist, select the Include Diabetes Education Checklist checkbox. The checklist will automatically be populated with the information entered in the Diabetes Education tab of the medication review.
